We have a fabulous job opportunity for you to join our awesome team as our Supported Living Coordinator. We support our brilliant customers who have complex mental health issues and challenging needs.
You’ll be based at our mental health recovery service working with the team to provide support to our customers. It’s not always easy and our customers can be tough to support at times, however like our team, we’re sure you’ll find the job truly rewarding.
We help our brilliant customers by supporting them in their homes to gain independence, and the skills needed to move forward in the right direction. Our customers are all unique, with diverse needs. This means we need to adapt how we support them.
You’ll help change our customer’s lives (and our services) for the better by undertaking risk assessments, support planning, goal setting and regular reviews.
As our Supported Living Coordinator, you’ll help our customers identify their aspirations, and work with them to create plans that help them achieve these, in collaboration with clinicians and wider NHS services.
Each plan will look to meet our customer’s individual potential and long-term goals, making sure they meet safeguarding and confidentiality requirements.
You might support with breakfast and their medication, attending college or the gym, taking part in community and social activities, as well as trips out, walking, and other meaningful activities to give choices based on their needs.
