We have a fabulous job opportunity for you to join our awesome team as our Waking Nights Support Coordinator. We support our brilliant customers who have complex mental health issues and challenging needs.
We help our customers by supporting them in their homes to gain independence and the skills needed to move forward in the right direction. Our customers are all unique with diverse needs, meaning we need to adapt to how we support them.
As our Support Coordinator, you’ll help change our customer’s lives (and our services) for the better by undertaking risk assessments, support planning, goal setting and regular reviews.
To help our customers identify their aspirations, you’ll work with them to create plans that help them achieve these, in collaboration with clinicians and wider NHS services. Each plan will look to meet our customer’s individual potential and long-term goals,making sure they meet safeguarding and confidentiality requirements.
In this job, you may support our customers with breakfast, medication, attending college or the gym and taking part in community and social activities. You may also help with trips out, walking, and other meaningful activities.
You’ll be based at our mental health recovery service working with the team to provide support to our customers. It’s not always easy and our customers can be tough to support at times, however this is a hugely rewarding job
