MAIN DUTIES Establish and maintain effective communication with patients, families and carers in planning and delivering compassionate care Develop and maintain appropriate communication with members of the multidisciplinary team Assist in planning, implementation, and evaluation (APIE) of individual programmes of care for patients using an appropriate nursing model Provide and deliver a high standard of comprehensive skilled and evidence-based nursing care recognising and meeting the specific needs of the patients Be able to prioritise their own workload Support and empower patients, carers, and their families to identify and achieve their health and well-being goals. Carry out clinical observations on patients, recognising, recording, and reporting abnormalities and changes in patients condition to a registered practitioner Assist in the recording of fluid balance, reporting discrepancies to a registered practitioner Assist with all aspects of personal hygiene for the patient Assist in ensuring patients nutritional needs are met Assist in planning and implementing methods of rehabilitation for patients and their carers Safely dispose of equipment, post procedure, restocking where appropriate Assist in the safe moving and handling of patients utilising appropriate equipment Utilise early warning score indicators (e.g., NEWS) to recognise any patient deterioration/sudden change in condition, reporting immediately to a registered practitioner Prepare and undertake interventions consistent with evidence-based practice, transferring and applying knowledge and skills to meet patients needs Maintain accurate, contemporaneous records that ensure safety and continuity of patient care. Utilise appropriate resources in the provision of total patient care. Acknowledge any limitations of competence in accepting care activities outside agreed limitations and inform a registered practitioner Undertake other relevant duties arising from the work of the Primary Health Care Team as requested by the District Nurse Team Manger and/or Lead District Nurse and/or Nurse Manager District Nursing
