All applicants must have a Level 3 qualification in health and social care.
To develop, co-ordinate and implement a rehabilitation plan with assigned users of the service in accordance with their identified needs and desired outcomes. To develop supportive and therapeutic relationships with residents in order to promote recovery. Key-workers will share good practice and promote positive attitudes in accordance with the Almond Villas ethos and the requirements of the Care Act 2014.
To work with allocated residents in a person -centred way, following the Recovery Star Model in order to develop, follow and review a rehabilitation programme in accordance with individuals’ desired outcomes.
To be involved in the assessment, planning, monitoring, implementation and evaluation of allocated residents’ recovery plans and safety assessments.
To support residents to consider previous risk factors including alcohol and drug use and identify relapse prevention strategies.
To support residents to develop daily living skills such as shopping, budgeting, household management and to meet their personal care needs as necessary, taking into account ability and promoting independence.
To support residents to manage their medication regime and support their progression to self-medication.
To support residents to understand and meet their healthcare and nutritional needs whilst respecting their individual choices.
To encourage, promote and support residents’ engagement in meaningful activities in accordance with their interests.
To encourage, promote and support residents’ engagement in their community including through voluntary work and education.
To plan and run therapeutic groups according to resident needs.
To hold weekly 1:1 sessions with residents with allocated users of the service in order to plan and review their rehabilitation programme, covering areas including daily living skills, activities and relapse prevention.
To liaise with team members regarding residents’ needs and goals on an ongoing basis including attendance at and contribution to team meetings.
To liaise with relevant professionals regarding allocated residents’ needs and progress.
To attend and contribute to Multi-disciplinary review meetings in relation to allocated residents.
To protect the confidential nature of the work.
To complete residents notes and monitoring records to a professional standard.
To complete and review assessments and recovery plans for allocated residents in accordance with the Recovery Star Model and Almond Villas’ protocols.
To contribute towards allocated residents’ safety assessments.
To produce reports for the purpose of CPA review meetings and other professional meetings.
To ensure that all other documentation including medication records is completed accurately
**Salary**: £10.00 per hour

Ability to commute/relocate:
- Blackburn, Lancashire: reliably commute or plan to relocate before starting work (required)
**Education**:
- GCSE or equivalent (preferred)
**Experience**:
- Mental health rehabilitation: 1 year (required)
- Support worker: 1 year (preferred)
Licence/Certification:
- Driving Licence (preferred)
