Care Coordinator

**Overview**
**Responsibilities**
- Serve as the primary point of contact for clients, families, and healthcare providers.
- Coordinate client care plans by scheduling appointments, arranging transportation, and ensuring timely follow-up on services.
- Maintain accurate and up-to-date client records in compliance with privacy regulations.
- Collaborate with multidisciplinary teams to assess client needs and develop comprehensive care strategies.
- Provide support and guidance to clients and families regarding available resources and services.
- Monitor client progress and adjust care plans as necessary to meet evolving needs.
- Facilitate communication between clients, families, and healthcare providers to ensure continuity of care.
**Qualifications**
- Previous experience in a healthcare or office setting is preferred.
- Strong organisational skills with the ability to manage multiple tasks effectively.
- Excellent communication skills, both verbal and written, with a focus on empathy and understanding.
- Ability to work independently as well as part of a team in a fast-paced environment.
- A genuine passion for helping others and improving their quality of life.
