We are seeking a motivated and dynamic Rehabilitation Assistant with a passion for general physical rehabilitation to work within NELFT’s London based Intensive Rehabilitation Service (IRS).

The IRS is a large multidisciplinary community team which offers an alternative to hospital bed-based rehabilitation, supporting patients to recover at home within their own environment. As part of the team within IRS, you will have a caseload of community patients and you will support the nurses, Physiotherapist and Occupational therapists in IRS to provide patient-centred treatments. There is an expectation to join the weekend and late shift (12-8pm) rota.

Probationary Period

At NELFT we set very high standards to ensure we provide quality services. We require prospective employees to carry out an online literacy and numeracy test, equivalent to NVQ Level 2. Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focussing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham Essex

**The main responsibilities of the post**:
1. To work flexibly and assist in effective and efficient delivery of the service.

2. To be able to conduct therapy and nursing treatment plans with people in their own homes. This includes chronic, acute and palliative care within own competencies, recognising own limitations and seeking advice when necessary.

3. To provide patients and relatives with information and education thus ensuring they have meaningful choices that promote dignity, independence and quality of life.

High Cost Area Supplement - London

This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (with minimum payment of £3,899 and a maximum payment of £4,968).
