The Health Inclusion Team provides an innovative nursing and health visiting service to a transient population. That is; people who live in homeless accommodation, domestic abuse refuges, anti-trafficking refuges, those seeking asylum and refugees (registered with the Mulberry Practice), Gypsies and Travellers.
As an Outreach Family Support Worker (OFSW), you will be expected to work in partnership with children, families, and individuals with complex needs to provide high quality family support. You will contribute to the development of the service for Homeless people, Gypsies and Travellers and Asylum Seekers and Refugees.

The post holder has a key role in offering practical and emotional support, with the aim of addressing health inequalities to the wide range of families registered with the Health Inclusion Team. These include Homeless, Gypsy and Traveller families; and families seeking asylum in Sheffield.

This service carries an annual fee, which employees are responsible for paying up front. However, employees may claim this cost back through the Trust's Employee Expenses Reimbursement Policy. This role involves providing direct support and advocating for individuals, children and families, on a citywide caseload with the aim of reducing health inequalities.
Our clients are often victims of domestic abuse, trafficking and/or seeking asylum. As well as being homeless/transient they often have complex issues around chaotic lifestyles, substance misuse, debt, mobility, mental health issues and child protection requiring a post holder with a high level of understanding and ability to work in a trauma informed way
