WELLBEING LINK COORDINATOR, COVERING CASTLE POINT, RAYLEIGH OR ROCHFORD, ESSEX

Castle Point Association of Voluntary Services (CAVS) is a not-for-profit charity and voluntary service organisation located in Benfleet Essex.

An exciting opportunity has arisen to join our Social Prescribing Team and work with professional and friendly colleagues from CAVS Head Office and/or remote working
within Primary Care Practices in Castle Point, Rayleigh and Rochford.

This highly rewarding role will involve working intrinsically with GP practices, their patients and the CAVS team. Assistance of a non-clinical nature will be provided to those with long-term health conditions, low level mental health issues, those who may be lonely and isolated, people who have lost confidence or have social needs; all differing issues that can affect an individual’s happiness and welfare. A holistic approach based on the person’s priorities and a focus on ‘what matters to me’ is key to the service delivery.

The Wellbeing Link Coordinator (known as a Social Prescriber) will have significant interaction with other agencies and the linking of people to community, voluntary and other statutory services for emotional and practical support.

If you have excellent communication skills, enjoy helping others and ideally have experience of working with vulnerable adults in healthcare, social care or other applicable settings, then we would very much like to hear from you.

Hours: 37.5 per week

**Salary**: £25,000.00-£25,240.00 per year

Schedule:

- Monday to Friday
