An exciting opportunity has arisen for an experienced Head of Community Services to join a community-focused care charity dedicated to supporting older people.

Morden College is in Blackheath, South-East London within its award-winning grounds. Founded in 1695 by the pioneering merchant, Sir John Morden, the Charity is dedicated to supporting older people and has been at the forefront of enriching older people’s lives for more than 300 years. Today the Charity’s core purpose is to provide older people who are need, with a home for life; to support them as they grow older, and to provide care services, including residential care and residential nursing care, if the need arises. We are a strong community, committed to enabling the older people we support to have the highest quality of life, for the rest of their life.

The Charity’s residential care home, Cullum Welch Court (“CWC”), provides personal nursing and dementia care 24/7. With 32 beds, it is small enough to maintain a personalised service to its residents, yet large enough to provide the facilities required to meet their range of needs.

**Purpose**

As a valued member of our management team, the Head of Community Services leads the staff working in the John Morden Centre, Health & Wellbeing Team, and Ralph Perring Court, ensuring effective use of resources and time, maintaining quality and safety, promoting high performance within a culture of continuous improvement, both individually and as a team.

**The Role and Your Responsibilities**

You will be directly accountable for the day-to-day delivery of high-quality and dignified support services promoting respect, equality, and the independence of residents.

**This will require you to**:

- develop Morden College’s approach to community engagement, use of communal space and resident involvement to support the community to live a ‘Good Life’ based on what matters to the individual.
- ensure that all staff adopt and maintain a person-centred approach to the development and delivery of high-quality services and uphold the principal that everyone is at the centre of our services and the support they require.
- lead the development of Information Technology Support and Assistive Technology for residents to promote digital inclusion, enhance their digital skills and help maintain their independence.
- implement a case management approach that proactively identifies individuals at high risk, assesses their needs, produces a personal support plan, and ensures co-ordination of the plan to enable people to live independently and safely.
- Ensure that a comprehensive housing management service is provided across the Almshouse Accommodation, be responsible for the provision of a safe and secure environment ensuring a range of tailored housing and associated services are available for beneficiaries to enable them to maintain their residency and live independently in the community
- Be accountable for the referrals and lettings process, ensuring effective strengths and safety assessments are carried out and that properties are occupied in accordance with Morden College policies, while making best use of available accommodation and minimising voids.
- Promote a culture of service to residents which encourages consultation, continuous feedback and involvement and ensure a range of opportunities are available to help them to shape the delivery of future services.
- In collaboration with the Head of Clinical Services be responsible for the development and implementation of a case management system to identify those individuals at high risk, assess their needs, develop a support plan, ensuring coordination of the plan to enable people to live independently and safely.
- Working in partnership with appropriate local agencies, lead a professional advice and guidance service for individuals living with the Morden College community aimed at enhancing their resilience, ensuring access to appropriate benefits and grants, and maintaining their wellbeing.
- Ensure that performance management arrangements are in place within accountability areas to measure the impact of service delivery and development on user groups.
- Lead the development of Information Technology Support for residents to promote digital inclusion and increase independence using assistive technology
- Build and grow the offer to our community, delivering high quality wrap-around activities alongside our events programme and establishing lasting connections with individuals and organisations.
