The post holder will support the Macmillan Prostate Cancer Nurse Specialist Team in managing patients on the Remote Monitoring pathway and be responsible for the administrative aspect of this service. They will provide general information help and support during the cancer pathway.

To work under the direction of the registered nurse providing high quality patient centred care to those living with stable prostate cancer.The role involves being responsible for aspects of direct care delivery, coordination of the patients' pathway, assessment of patient/carer needs and the provision of general information, help and support to those living with stable prostate cancer.To be an extended member of the Urology MDT, ensuring effective communication with the team, across the County, Primary Care and voluntary agencies.

To support the administrative aspects of the service.

Provide effective input into the work of the Trust, to support the achievement of our vision and values
