Our fabulous Urology Specialist Nursing Team are excited to offer this newly created Band 6 post to support patients on their Cancer Pathway journey, ensuring their emotional, psychological and physical health needs are met. This role is part of the Urology multidisciplinary team and works closely with Macmillan resources and patients and their families. Overview This core member will provide high‑quality, patient‑centred care to patients and carers living with prostate cancer from diagnosis through to treatment and beyond. The post holder will be the main point of contact for a caseload of patients, working with primary and secondary care providers, the MDT and external partners, ensuring a seamless, holistic service across Northampton and potentially Kettering sites. Responsibilities Provide specialist information, advice and support to patients & carers from diagnosis onwards. Act as main point of contact and coordinate the care pathway for a caseload of patients. Support Prostate Clinical Nurse Specialists in delivering an innovative, high‑quality service to prostate cancer patients. Implement key elements of Personalised Care including Holistic Needs Assessment, End of Treatment Summary, and Health & Wellbeing opportunities at appropriate stages. Conduct Holistic Needs Assessments using recognised tools – identify physical, emotional, social, financial and spiritual needs. Use supportive conversations to develop, implement and review care plans, adapting to changing clinical situations. Attend the weekly specialist multidisciplinary team meeting, contribute to discussions and coordinate MDT actions. Review emergency admissions, work with the admitting team to agree and implement management plans. Advise on symptom management, refer complex needs to the Macmillan Specialist Palliative Care Team. Document outcomes in hospital notes and on the Somerset Cancer Register, demonstrating enhanced nursing practice. Provide Level 2 psychological support and refer complex cases to the Macmillan Clinical Psychologist. Support patient and carer communication, act as advocate during significant consultations, and coordinate the care pathway. Ensure palliative care needs are identified and meet referrals for specialised services. Identify patients’ Preferred Place of Care, facilitating discharge where possible. Support patients under the AMBER Care Bundle pathway. Promote a culture that encourages self‑management and use the dedicated cancer You‑Tube channel. Work in line with the NMC Code of Professional Conduct and promote collaborative relationships across care boundaries. Provide nursing advice and support to other health professionals regarding prostate cancer care. Qualifications Qualifications requirements are not specified in the posting.
