Care Coordinator

We are looking to recruit a Care Coordinator to work with our team in our Gorleston Office.
Full, clean driving licence required. Experience in care and coordinating care visits is essential. Qualifications are not essential but desirable.
**Duties**
- To work as part of a designated team
- To discuss with the Registered Manager Service Capacit
- To ensure agreed care plans are implemented, identifying progress towards agreed targets.
- To be responsible for Service Users and Care Support Workers within a designated area.
- To delegate work in accordance with the agreed care plan.
- To co-ordinate time in & out of the Service Users home and ensure correct paperwork is completed.
- To keep up to date records and reports to file manually and electronically.
- To attend meetings and reviews on care plans when required by outside agencies.
- To visit Service Users in their own homes as required and to complete documentation.
- To liase with G.P and pharmacys regarding Service User's needs.
- To work closely with our full time co-ordinators
- To carry out duties in accordance with the Company Code of Conduct.
- To promote Company Policies and Procedures.
- To attend regular in-house meetings with the management outlining any problems, concerns or changes in care plans.
- To carry out on-call duties on a rota basis. (there is additional pay for theses duties)
- To carry out care duties as deemed necessary or appropriate as part of emergency contingency plan
- To ensure all relevant training is up-to-date.
Pay rates and additional on-call rates will be discussed at interview.
**Job Types**: Part-time, Contract, Permanent
Pay: £12.50 per hour
Expected hours: 15 - 20 per week
