Our independence at home service offers much needed, practical short term support for people who have either just been discharged from hospital or to prevent them needing to be admitted into hospital. We will carry out a full assessment of the persons' needs before your visit. We want to make sure the client settles back in and has everything they need to feel safe and secure, and can continue to live independently at home.

We are looking for a team of dedicated volunteers to support older people as they are discharged from hospital.

You would be asked to support clients in their own homes or on their physical journey home from hospital, we ensure the person needing help lives somewhere close to you and we'll pay your mileage costs to and from the client.

Volunteers typically help with the following things:

- Food shopping
- Light housework to include changing beds, hoovering and other light tasks.
- Encouraging clients to become independent to ensure they can stay living in their own homes
- Befriending to encourage their confidence
- To be aware of any other support the clients may need and report back to a team leader so we can arrange this.
- Transporting the client

Volunteers will not need to provide any personal or medical care.

**What is required of me?**
- Our volunteers need to have a patient and understanding attitude
- We welcome volunteers for as much or as little as you are comfortable with
- Don’t worry if you are not well or are on holiday, we can always arrange cover for you.
- We will arrange for a DBS check prior to you undertaking any visits

**Job Types**: Part-time, Permanent, Freelance, Zero hours contract
Contract length: 1 - 12 months
Part-time hours: 3 per week

**Salary**: From £0.01 per hour
