1.Working closely with the Dementia Operations Team Leader, enable the smooth running of the Dougie Mac 1:1 Dementia Support Service to meet organisational needs. 2.Coordinate new referrals into the 1:1 Dementia Support service, and act as the first point of contact for new referrals wishing to access the service. 3.Act as the first point of contact for Dementia Support Workers, coordinating calls as required and allocating Dementia Support Workers to service users appropriately. 4.Undertake assessments of new and current service users making individualised judgements to ensure suitability for the service.
Assessments can take place onsite at the Dougie Mac Adult Hospice or in the community. 5.Create and implement a plan of care and support for the service users. 6.Working with the Dementia Support Team Leader, create ongoing rotas for the 1:1 service, ensuring all calls are covered during staff sickness or annual leave as required. 7.Ensure clear communication with service users and their carers on the service.
8.To manage the flow of service users in and out of the service. To escalate and reassess appropriate should a service users needs alter. This may include liaising with internal and external health care professionals. 9.Maintain strong positive links with relevant bodies and agencies, including social workers and adult social care services, to enable effective support for service users.
10.Working with the Dementia Operations Team Leader, contribute to the effective recruitment and induction of new staff as required. 11.Monitor relationships between service users and Dementia Support Workers effectively and assist in identifying any issues and reaching suitable solutions as required. 12.Establish and maintain good relationships with the patient, carers and all health care professionals. 13.Gather feedback from service users and carers to contribute to our commitment for service improvement and quality assurance.
This may involve the creation of reports to team leaders. 14.To lead with changes in process and act as a role model for change. 15.Attend to the personal care needs of individuals in line with agreed care plans, respecting the persons dignity at all times. This may involve some moving and handling, in line with the organisations policies.
16.Provide transport for service users using own vehicle (appropriately insured). 17.Attend and contribute to supervision and reflective practise sessions regularly and attend staff meetings. 18.To ensure patient confidentiality is always maintained and information obtained during the course of employment is not released to anyone other than those acting in an official capacity. 19.To work as an integral part of the multidisciplinary team, ensuring that the needs of patients and carers are identified within a holistic framework
