The Lincoln Older Adult Community Mental Health Team have a new and exciting opportunity to join the team in a Band 7 Leadership Role. This role is to provide and support the team with clinical expertise and to develop a service to meet the needs of the older adult population in the Lincoln and Gainsborough Locality. We require someone to lead the team to deliver of high-quality evidence-based care to older adults experiencing both functional and organic mental health problems. This role is a second band 7 within the Lincoln and Gainsborough Team.
The team is based at Lincoln and maintains excellent communication and close working supportive relationships with the wider OA CMHT across Lincolnshire. There is active involvement in Neighbourhood Teams and Place Based Teams as part of the Community Transformation Agenda.
We are currently reviewing our service model and this role will be a key leader in our next steps of clinical, organisational change and delivery. The post holder will have a sound clinical knowledge to support this change and support us in shaping our services to enhance patient and carer experiences.
We are seeking a leader who has a mental health nursing, social work or AHP qualification. In addition, you will be able to demonstrate experience required to support this role and an understanding of the agenda’s relating to Mental Health Care Needs of the Older Person for mental health and Memory Pathways within the service.
The post holder will support the Service Manager in providing effective frontline management of the multi-disciplinary community Mental Health team. In agreement with the Service Manager, the post holder will provide leadership and, in working in partnership with users will ensure access to the full range of specialist assessment and interventions.
Communicating with families and carers is a key part of this post. Ensuring that the team are involving families and carers in the patient care plans and care delivery is especially important. As the team coordinator you will have excellent listening skills and this skill will need to be applied to all the people that you are in contact with.
You will lead a team that comprises of Consultant Psychiatrist, junior doctors, senior mental health practitioners, AHPs and a Dementia Support Service. This will include taking responsibility for managing resources within defined budgets, managing performance indicators and supporting the clinical team to provide and evaluate programmes of care to ensure that our patients and carers are receiving the level of care required by our service.
