The post holder will be responsible for assisting in the management and care of patients based upon individual needs, under the guidance of the Nurse in Charge and in providing practical support to patients in performing day to day activities, guidance and education.

The post holder will also act as a mentor to students and apprentices, and be responsible for supervising unqualified staff. The post holder will be expected to work collaboratively with key partner organisations, patients, carers, clinicians and allied health practitioners within the multi-disciplinary team in delivery services; provide a 24 hour service, 7 days a week.
In delivering the post holder duties you are expected to display behaviours that are aligned with the Trust values and as required in accordance with the NHS Constitution at all times.

An exciting opportunity has arisen to join our service as a Band 4 Associate Practitioner at the Derwent Centre, Harlow.

The post holder will have the responsibility in developing their expertise in physical health needs, devising care plans and supporting junior support workers. You will act as a role model to other colleagues within the team. You will work collaboratively with key partner organisations, service users, clinicians and other practitioners within the multi-disciplinary team.

You will deliver high quality, effective operational service to people with mental health needs, in accordance with the principles and objectives of the Trust and its vision as identified in the service. You will be responsible along with other members of the team for the implementation of all care aspects of individual care planning and attending to the physical and psychological needs of patients under the care of the nursing team.
