We are currently looking to recruit a motivated and enthusiastic Band 6 Mental Health Practitioner to join our Cambridge North City NT- OPMH services.
We are part of the exciting Integrated Care model built around NTs. We provide patient focused, evidence-based mental health interventions for people aged 65 and above, with both Functional and Organic mental health illness.
You will have the opportunity to manage a caseload of service users, delivering a range of clinical skills and therapeutic and recovery-based interventions.
This service covers Cambridge North Villages; however, you may be required to support colleagues and service uses in other areas if need arises. Therefore, successful applicants will need to be able to travel independently and in a timely manner.
- Mental Health practitioners in Neighbourhood Teams will provide assessment and interventions for people with significant mental health conditions
- Offer training, advice and guidance to colleagues in the neighbourhood team on common mental health conditions.
- Participate in Neighbourhood Team MDT meetings and ensure key information is recorded in shared care plans
- Work closely with the mental health integrated care team in order to provide an excellent mental health service.

1. Assessment of mental health needs
2. Delivery of mental health interventions for both service users and their carers in accordance with mental health care pathways for dementia and functional illness.
3. To supervise and assess the use of prescribed medications according to professional guidelines and policies. To be familiar with therapeutic and side effects of prescribed medication and observe statutory and local policies in the prescribing, storage and administration of medications.
4. To practice as a supplementary non - medical prescriber (with appropriate qualification) providing specialist advice in non - medical prescribing and medicine management policy line with Trust’s non medical prescribing policy.
5. To develop the professional role within the neighbourhood teams and To offer guidance and support to colleague within the neighbourhood teams
6. To offer training and development of skills in working with common mental health conditions within the neighbourhood team
7. To contribute to local multi-disciplinary team reviews and care planning for identified service users
8. To provide support to people on discharge from hospital
9. To work closely with colleagues in the mental health integrated care team and the Crisis Resolution and Home Treatment to ensure smooth transitions of care and joint work.
10. Ability to work flexible including working outside core hours if clinically required.
