An exciting opportunity has arisen for a full time Band 3 HCSW rehabilitation assistant working within the specialist Pallative care Multi
- disciplinary team, on the inpatient unit in Ty Olwen
The successful person will be based with the nursing team within Ty Olwen and will work alongside the therapy team in TY Olwen IPU and will be supporting the current team of Nurses and HCSW with their expanding service.
The post will be working between the hours of 9am - 5pm Monday to Friday
**Job Summary/Job Purpose**:
- The HCSW Rehabilitation Assistant is a team based worker who delivers
delegated care/tasks as part of a multi-professional team delivering person centred care that focuses on enabling patients to achieve their rehabilitation potential and maintenance of their maximal level of function as dictated by their condition.
- The post holder will be an integral support to therapists and nurses in
supporting the assessment and provision of rehabilitation to patients.
- The post holder will be required to work unsupervised, undertaking
independent interventions and decision making within an agreed framework,
although supervision and support is readily available from the registrants.
The role is based within both the in-patient and community settings working flexibly in order to provide rehabilitation programmes and equipment for
patients both in hospital and in their own homes.
- The post holder will provide the highest standards of safe and compassionate care that maximise patient recovery and rehabilitation potential and will play an integral role in developing and supporting the culture of a whole team approach to promoting the fulfilment of maximal patient independence.
