**About The Role**

Bradford City is the fourth largest Metropolitan District in the country and home to a population of around 534,300 residents. Bradford has the largest percentage of population under the age of 16 outside London.
Our Targeted Family Support is an essential part of a local network of services working alongside partner agencies and families to deliver good, quality family support. Children are at the centre of everything we do, and our focus is to improve outcomes for children. We are looking to recruit friendly, dynamic, and enthusiastic individuals to join our team and we want to hear from you if you:

**Key Responsibilities**
- To undertake or contribute to holistic and strength-based assessments of need and devise and implement appropriate plans for babies, children, young people, and families.
- To ensure the appropriate participation of children and young people in their own assessment, plans, and reviews.
- To manage own caseload and co-ordinate multi-agency support to deliver tailored family interventions and support which tracks and shows progress over time.
- To arrange and chair reviews on a regular basis to track and show progress, including children, young people and families and partner agencies as appropriate.
**About You**
- Minimum of 2 years continuous and recent experience of working directly with children young people and parents including evidence of recent case experience.
- NVQ 3 or Diploma 3, in Health and Social Care, including knowledge regarding children’s development, or equivalent child related qualification.
- GCSE English and Maths or equivalent.
- IT literacy.
- An understanding of Safeguarding and promoting the welfare of children and young people.
