Teqniq is searching for a **Care Co-ordinator**to work in the public sector.

35 hours per week.

09:00-17:30

We are looking for individuals who are passionate about working with people who have learning disabilities and their families. If you are committed to supporting people with learning disabilities to live autonomous lives and achieve positive wellbeing, then we want to hear from you.

This is an exciting opportunity to be based within a local authority integrated health and social care service. The service comprises of many disciplines including Social Work, Community Nurses, Psychologists, Occupational Therapists, Speech and Language Therapists, Art Therapists, Physiotherapists and Psychiatrists. The Care Coordinator post will sit within the social work team.

You will need excellent communication skills, both verbal and written. The Care Coordinator will be required to attend multi
- disciplinary meetings and present cases, communicate effectively with service user’s and their families as well as a range of internal and external colleagues and partners. In addition, the Care Coordinator will be required to write accurate, comprehensive support plans and assessments.
