**Job Title**:Care Coordinator

**Hours**: 35 hours per week
**Pay**: £19.93 per hour (umbrella), £16.30 per hour (PAYE)

Seven Resourcing is seeking a Care Coordinator to deliver high-quality assessment, care management, and social work services for individuals and carers. This role involves working face-to-face with service users, families, and external partners to ensure support packages meet individual needs in a person-centred and strength-based manner.

**Key Responsibilities**
- Assess and manage care needs in accordance with relevant legislation, policies, and guidelines.
- Conduct face-to-face meetings with service users, families, and partners.
- Review, recommend, and amend support packages to ensure they meet service user needs.
- Collaborate with internal and external stakeholders, attending multi-disciplinary meetings.
- Write comprehensive support plans and assessments.

**Requirements**:

- Passion for supporting individuals with learning disabilities and their families.
- Commitment to promoting autonomy and positive well-being for service users.
- Excellent verbal and written communication skills.
- Ability to present cases effectively in multi-disciplinary meetings.
- Strong organisational skills and attention to detail in creating support plans and assessments.
