Colorectal Cancer Care Coordinator

The Salford colorectal specialist nursing team are looking appoint a knowledgeable and enthusiastic individual, with experience of working in cancer care or a healthcare setting, to join their experienced team supporting the delivery of high quality personalised cancer care.
The cancer care coordinator will play a key role in supporting the delivery of personalised care and support for colorectal cancer patients. They will undertake Holistic Needs Assessments, complete the associated care and support plans and provide wellbeing contacts for patients on a supported self-management follow up pathway.
The post holder will be required to provide general information and support about cancer and cancer services to enable navigation of health and social care and to make choices that are best for the individual’s cancer and their life.
The colorectal specialist nursing team provide a high quality, evidence based, service to support patients and those important to them as they navigate often complex care pathways. We strive to ensure our services are accessible and along with a multidisciplinary team provide psychological, practical ,emotional, social and financial support to patients and their relatives throughout the cancer pathway.
The Colorectal Cancer Care Coordinator will work as part of the colorectal specialist nursing team to ensure patients receive high quality personalised cancer care. They will be expected to work with all grades of medical and nursing staff and wider health / social care and community teams (including third sector and charity).
Completion of Holistic Needs Assessments, completing the associated care and support plan
Providing wellbeing contacts for patients on a supported self-management follow up pathway.
Phlebotomy for CEA blood monitoring clinic
Provide general information and support about cancer and cancer services to enable navigation of health and social care and to make choices that are best for the individual’s cancer and their life.
The post requires excellent communications skills, compassion, and a genuine interest in supporting patients who have colorectal cancer.

Main Tasks & Overview of Responsibilities
- Develop and maintain effective working relationships with clinicians, managers, others within the department and wider organisation.
- Provide support to people affected by cancer, often dealing with highly emotional and sensitive issues and have an ability to manage individual situations effectively.
- Undertake Holistic Needs Assessments for cancer patients and produce a care & support plan.
- Complete any follow up work generated by holistic needs assessment & care planning, e.g. making onward referrals.
- Independently run the CEA blood monitoring clinic.
- Make pre-planned wellbeing telephone calls to patients to assess on-going needs and to enable a proactive prevention approach
- Where applicable, ensure access to information and education resources to support decision making.
- Liaise with Clinical Nurse Specialists, other Healthcare professionals and cancer services as appropriate.
- Liaise with patients, relatives/carers and Macmillan Information and Support Team and community organisations.
- Provide basic telephone advice and refer on/signpost to other sources of support.
- Assist with the planning and delivery of health & wellbeing events
- Take an active part in team meetings and audits as required.
- Prioritise own workload.
- Provide emotional support to patients and their relatives/carers and escalate concerns to the specialist nursing team where required.
- Participate in the quality assurance panel.
- Develop own skills and knowledge and contribute to the development of others.
- Monitor and maintain health, safety and security of self and others in own work area.
- Assist in maintaining and developing services.
- Ensure own actions promote quality and alert others to quality issues
- Support people’s equality, diversity and rights.
- Assess the health and wellbeing needs of people whose needs are relatively stable.
