We have an exciting opportunity for a **Resettlement Worker - Community Services Centre **to join our team based in **London**. You will join us on a **full time, permanent** **basis** and in return, you will receive a competitive salary of **£29,378 per annum (NJC Scale 15-19) inc LW + pension and other benefits.**

As we emerge from the pandemic, the Passage has worked hard to review how we deliver services with an important focus on reflecting our learning in how we support the people and communities we work with.Our new service model focuses on prevention, a renewedsense of urgency to end homelessness, collaboration, ambition for the people we work with and advocacy for social justice.
**About the Resettlement Worker - Community Services Centre role**:
You will provide a warm welcome and safe environment for our clients which is responsive to their needs; ensuring that the initial engagement with our clients is carried out in a positive, helpful manner and are delivered with enthusiasm. You will work withclients to explore solutions and opportunities to resolve their housing situation.
Working closely across the Community Service Centre teams, you will ensure the delivery of the best possible service to our clients and work positively within the community with partner agencies to deliver advice and create resettlement plans within a communityoutreach environment.
You will work within a No Night Out service model which entails intensive one to one case work to resettle the people we work with into a secure and sustainable accommodation and carry out regular follow up work to ensure tenancies are sustained.
**Key responsibilities of our Resettlement Worker - Community Services Centre will include**:
- Conducting thorough, needs led assessments and co-produce informed, accurate and effective Resettlement plan
- Working in partnership with our specialist advice services to assist our clients to obtain their welfare, housing, and related rights by employing a detailed knowledge of relevant legislation (eg Housing, social care and immigration)
- Providing appropriate advice and support to enable individuals to improve their health and wellbeing
- Working in partnership with external agencies, using a multi-disciplinary team approach, to achieve the best possible outcomes for our clients
- Assessing factors which could impact on the safety of clients or colleagues, and developing a management plan with the client and other services delivering relevant support, communicating these plans in line with organisational requirements
- Supervising volunteers as appropriate
- Maximising effective teamwork across teams, supporting colleagues/peers and volunteers in all aspects of their work
**Experience of**
- Undertaking comprehensive assessments to assess support needs and interventions required
- Providing resettlement support to individuals who have experienced homelessness
- Working within a team and developing strong and effective relationships with others to achieve organisational aims and objectives
- Accurate and effective record keeping using case management databases
- Building and developing effective relationships to support vulnerable, disadvantaged or socially excluded groups
- Collaborative working across multi-disciplinary teams to achieve positive outcomes for an individual
**Knowledge of**
- Working within a psychologically informed framework to support vulnerable individuals
- Welfare rights and entitlements and how they impact on our client group
- The Homelessness Reduction Act and how this applies to local authorities and accessing housing
- An understanding of the issues involved in resettling clients and the benefits and challenges of reconnection
