**TA - Senior Trusted Assessor**
Venn Group is currently working with a large NHS company who are looking for a Senior Trusted Assessor. Below is a brief synopsis on the role;
**Duration: Start date - Nov - End of Dec**

**Banding: 7**

**Location: Southampton but will be required to work out in the community and at the trust**

**Rate: Daily PAYE £113.72 + £13.73 Holiday Pay = £127.45 or £140.20 Umbrella or PAYE £190.88 + £23.04 Holiday Pay = £213.92 or £238.61 Umbrella**

**Hours: 37.5**

**Day to day duties include**:

- The aim of the role is to support and facilitate discharges from the trust to residential and nursing care homes, within the city and environs
- TA role carries out independent assessments on behalf of care and nursing homes to help speed up the discharge process
- The Placement Team contact the provider to establish permission to request an assessment from the Trusted Assessor on their behalf
- Care Home Select receive referrals and forward on to the TA to complete assessments on the care home providers behalf
- Care home providers directly contact the TA themselves
- Direct requests from CHC or Deputy Director of Nursing

**Essential Skills for the role**:

- CHC nurse or senior social worker
- Knowledge of both acute hospitals and out in the community
- Ideally have some line management experience as we are recruiting for 3 more band 6 TA’s
- To ensure the proper assessment, management and mitigation of risk, including Information Governance, Health and Safety and Business Continuity
- To be responsible for recognising a disclosure of a concern from an individual that requires a safeguarding intervention to be made, and to take appropriate action
- To monitor safe practice and ensure that intelligence is fed back to the appropriate person in a timely manner
