Spring Social Prescribing works with individuals in Wellingborough and East Northants, who have long term health conditions and are seeking support from a link worker to live the very best life possible - we know this is an ambitious goal, but the rewards can be lifechanging for those concerned and this is certainly a job you can be proud of.

Our skilled staff team make a real difference to the lives of people living in Wellingborough and East Northants.

As a Social Prescribing Link Worker your role will be a combination of social prescribing, life coaching and connecting clients to local groups and opportunities in their community.

**About the role**

You will focus on improving their wellbeing by understanding what really matters to them and linking them to the right support, service or community activity. You will also have the opportunity to coach clients on a 1:1 basis, help them develop coping skills and gain confidence, with the aim of improving their physical and mental wellbeing.

You will be involved in supporting clients going to groups that we locate in their community and also groups that we set up and run ourselves be that craft, walking, wildlife, Tai Chi or art

We work with clients for up to 12 months, so have the privilege of capturing their view of their world at the beginning of their journey and again at the end to identify and share their outcomes and improvement.

We are seeking someone who is open-minded, non-judgmental, respectful attitude with an ability to easily create partnerships based on trust. The role can be challenging but incredibly rewarding, so you will need to be resourceful, creative, highly organised, IT competent with the ability to change and manage a varied workload independently.
