We are looking for an enthusiastic and forward thinking individual to join our multi-disciplinary team to assist with everyday activities and coordinate patients through the service as a secondment/fixed term contract for one year. The length of contract may be extended depending on further service funding.

The service has been developed in response to the Post Covid Syndrome/ Long Covid national guidance to provide assessment and rehabilitation to patients identified as having Long Covid. The team comprises of a clinical lead, admin, allied health professionals, GP and psychology.

This role will work alongside members of the multi disciplinary team to further develop and deliver a service for patients with Long Covid Syndrome across Gloucestershire. You will support patients through a variety of means including virtual, telephone or face to face, in individual or group formats.

These patients will need support with both physical and mental health wellbeing. There is an emphasis on delivering patient education to enable self-management, rehabilitation and referral onto other services to meet patients' individual needs.

The role will involve coordinating the MDT meetings and clinics, liaising with members of the MDT and different health care professionals, including a range of outside agencies. You will need excellent communication skills, be able to work independently and be motivated to support the MDT to deliver the patients rehabilitation goals to a high standard.

Training and support will be offered to those who may not fit all the criteria but may have transferable skills.

This post requires the post holder to be able to undertake all statutory and mandatory training and post holders must be able to undertake physical interventions. Fitness for physical intervention will be assessed by Occupational Health.
