We are looking for an experienced Care Coordinator to fill our vacant position.

Our Service Users are aged 18 to 65 and above. They have a range of care needs from simple companionship to assistance with personal care and medication.

Our Care Coordinators role is challenging but rewarding for the right person.As a Care Coordinator working for You & I Care, you will share our core values of Caring, Compassion and improvement. The role of a Care Coordinator is one of the most crucial roles of outstanding care provision.

**Job overview**
- To provide leadership to care staff within You & I Care Ltd.
- To provide care in accordance with current best practices, according to policy and procedures, agreed standards, legislative requirements, relevant regulations under the direction of the Manager
- To supervise the care delivery within the service
- To maintain skills at a current level, and undertake such training and development as may from time-to-time be required to maintain practices as up-to-date

**Responsibilities and Duties**
- Ensure Service Users are at the heart of the care delivery and their wishes and preferences enhance their wellbeing.
- To ensure that all staff contribute to the efficient running of You & I Care Ltd and supervise staff.
- To organise and co-ordinate care delivery in accordance with current best practices, according to policy and procedures, agreed standards, legislative requirements, and relevant regulations under the direction of the Manager and within the financial plans
- Ensure that the care service is delivered to Service Users at the times and for the duration specified in the relevant individual agreement(s) and contract standards. Uploading new Service Users/Carers onto the care management system, combine and organise Service User orders and checking availability.
- Be responsible for promoting and safeguarding the welfare of those individuals they support.
- Develop effective working relationships with other employees within You & I Care Ltd.
- Work in cooperation with members of the multi-disciplinary teams to maximise opportunities for Service User.
- Support the registered manager with the daily running of care provision.
- Assist with recruitment and staff management.
- Plan and schedule training needs, supervision and holiday requests.
- Negotiate availabilities and work schedules with Carers.
- Communicate all changes to Carers and Service Users.
- manage care planning, assessments and reviews.
- work in partnership with all the professionals.
- Auditing and care compliance
- Seek opportunities for personal and professional growth
- Attend statutory training and any other training as directed by the management

- Hold a fulling driving licence and have access to their own car.
- Proficient Written Skills
- Ability to induct and orientate new employees to the job role and service
- Great communication skills
- Be able to recognise the importance of effective communication within the multi-disciplinary team, with Service Users and their families/friends, whilst recognising peoples needs for alternative methods of communication and respond accordingly.
- Excellent telephone manner is required to deal with staff and Service Users under pressure.
- Be able to adapt and address situations quickly
