We're looking for an experienced Project Worker to support at a new service opening in Acklam, Middlesbrough. This role is a solo placement supporting a vulnerable adult to maintain their tenancy and learn new skills within the community.
**Job title: Project Worker (Supported Living)**
**Hours**:Full-time 40 hours per week, the shifts are 8am-8pm on days and 8pm to 8am on nights but there will be some flexibility needed for these shifts.
**Location**: Acklam, Middlesbrough (TS5)
**Salary**:£13.92 per hour.

A five-bedded residential care home in Middlesbrough committed to empowering adults with autism, complex needs, and challenging behaviors.
At Acklam House, we provide comfortable living spaces, emphasising community integration and skills development. Each person we support unique journey is supported through therapeutic interventions and engaging activities, encouraging personal growth and development.
**About the role**
The role of the Project Worker is an opportunity for a pro-active person to join an evolving Care Company. We are looking for a dedicated and empathetic person. The support team will provide practical assistance and emotional support to empower the people we support to achieve their goals and aspirations.
The team will be inducted and trained in anticipation of the person we support moving in. The person will require support around their daily living skills, health needs and will have a diagnosis of learning disability and or autism.
We require a Support Team who is empathetic and non-judgmental, someone who has experience in behavior that can be challenging.
- Support the individual with their own choices and help them to lead a meaningful life as per their wishes.
- Acting as a Key worker and understanding that ‘working together works best for a number of service users ensuring that the team always works in the best interests of the individual and encouraging positive behaviours across the team.
- Ensuring the individuals needs are met to maintain health and well being, physically, mentally, and emotionally. Advocate on behalf of the individual and ensure no decision is made without them.
- Recording and evidencing outcomes in service user records, ensuring confidentiality. Contributing to the preparation, implementation, monitoring and regular review of individual care and support plans. Giving the individual a voice that is heard and their aspirations are worked towards with them.
- Ensuring individuals’ dignity is maintained at all times and given support with their personal and intimate care as required in their care plan. This may include bathing, toileting and assistance with dressing.
- Medication management to include, ordering, storing and administering.
- Undertake service responsibilities in relation to completing audits related specific to supported living.
- Encouraging the individuals you support to engage with activities they already enjoy, and to seek out new experiences, Actively involve them in making decision about everyday tasks that will enhance their independence and engage with the wider community.
- Empower individuals to actively achieve their goals by taking time and communicating in their preferred method, be aware of the individuals’ beliefs and cultural needs and work in an open and trustworthy manner.
- To contribute to at least 6 team meetings each year, actively engaging in supervision and appraisal to continuously improve and develop knowledge and skill that will enhance the lives of the people within the service.
- To actively participate in training opportunities, attending all core skills and mandatory training to reach the required level of competence and utilising new skills to continuously improve and develop.
- Be alert to and proactive in safeguarding the health, safety and welfare of the people supported, yourself and colleagues based in the service, and escalate concerns in a timely, professional and appropriate manner.
- Undertake lone working responsibilities as per the needs of the service.
- Having a driving licence is desirable.
**Qualifications**
- Ability to demonstrate equivalent of Level 2 in English.
- Willingness to work towards Care Certificate.
- A good standard of general education.
- 2 years of working within a team.
- Experience in a care environment
- Experience, either paid or voluntary supporting people with learning disabilities, autism or mental health needs.
