**Job Overview**:
As a Care Coordinator, you will be instrumental in delivering high-quality care services to clients. You will work closely with the Registered Manager to plan and organize care services, ensuring client needs are met efficiently and in alignment with their care plans. Your responsibilities will include scheduling and allocating care staff, monitoring rotas, conducting client assessments, maintaining accurate records, and ensuring effective communication with clients, their families, and the care team. In addition, you will conduct competency assessments and spot checks, collaborate with a multidisciplinary team, and assist with filling in shifts to ensure continuous care delivery.

**Key Responsibilities**:

- **Care Planning and Coordination**: Work with the Registered Manager to develop and implement personalized care plans that address each client's unique needs and preferences.
- **Staff Scheduling and Shift Allocation**: Allocate care staff to client visits, ensuring continuity of care, adherence to care plans, and compliance with working time regulations. Fill in shifts as needed to ensure adequate coverage and consistent care delivery.
- **Monitoring Rotas**: Oversee and adjust staff rotas to ensure that service delivery remains efficient and responsive to clients' needs.
- **Client Assessments and Reviews**: Regularly assess clients’ needs and review care plans to ensure they remain tailored and responsive to their evolving requirements.
- **Competency Assessments and Spot Checks**: Conduct regular competency assessments of care staff to ensure they possess the necessary skills and knowledge. Perform spot checks to monitor care standards, address any issues, and ensure care is delivered to a high standard.
- **Collaboration with Multidisciplinary Team**: Work closely with the wider multidisciplinary team (including health professionals, social workers, and other stakeholders) to coordinate client care and ensure a holistic approach to service delivery.
- **Communication**: Serve as the main point of contact for clients, their families, and care staff, ensuring clear communication and prompt resolution of concerns.
- **Staff Support and Training**: Provide ongoing guidance and support to care staff, facilitating training and development to ensure high standards of care are maintained.
- **Recruitment and Onboarding**: Assist with the recruitment and onboarding of care staff, ensuring that new hires meet recruitment standards and are properly integrated into the team.
- **Record Keeping**: Maintain accurate and up-to-date documentation regarding client care, assessments, staff schedules, and coordination activities, adhering to organizational policies and regulatory requirements.

**Requirements**:

- **Experience**: Proven experience in care coordination or a similar role within the domiciliary care sector.
- **Organizational Skills**: Strong organizational and time management skills to manage client care schedules, staff rotas, and shift allocations effectively.
- **Communication Skills**: Excellent interpersonal and communication skills to build rapport with clients, families, and care staff.
- **Compliance Knowledge**: Knowledge of CQC regulations and compliance requirements is desirable.
- **Multidisciplinary Collaboration**: Ability to work effectively with a multidisciplinary team, coordinating care across different professionals.
- **Empathy**: A compassionate and person-centered approach to care, ensuring that clients' needs are met with dignity and respect.
