The Discharge 2 Assess Pathway 1 Service was created to ensure any person in a Lincolnshire hospital who is able to return straight home from hospital does so.The service provides short-term Assessment, Recovery, and Rehabilitation support, working collaboratively with Lincolnshire County Councils commissioned reablement provider with the goal of moving people to being as independent as possible. This will aim to enable people to return to their own home sooner from a hospital stay.

This service will follow the 'Discharge to Assess' (D2A) principles where a person is assessed for their long term needs in their own home or place of residence.

This service covers the whole county although people will generally work within a more locally based teams. This means that roles are available across Lincolnshire. The hours of the service are set to be 0800 to 2000, 7 days a week. We are able to consider part time working.

To work as a member of the Discharge to Assess Service delivering person centred care with particular focus on clinical and rehabilitation interventions, enabling individuals to achieve their optimum physical, psychological, social and emotional wellbeing.

To implement care as defined within individualised patient care plans, as directed by the senior colleagues, to patients in a variety of care settings within the community. To undertake assigned tasks supporting effective team working including elements of direct care, rehabilitation interventions and administrative tasks as delegated by senior staff.

LCHS recognises the increasing demand for more flexibility in working practices in order to accommodate the personal goals and commitments that employees experience at different stages during their working lives. Flexible working opportunities such as part-time working, compressed hours, job shares, and hybrid working models can help staff to balance their personal and work commitments. Arrangements can be put in place if it can be shown that the needs of the individual can be balanced with those of the service and the impact on other team members, enabling the Trust to recruit and retain skilled staff, raise staff morale, reduce absenteeism and respond to changing service needs more effectively.

To uphold the Code of Conduct for Healthcare Support Workers and Adult Social Care Workers in England at all times.

To provide person-centred clinical care under the delegation of a registered healthcare practitioner, working within own sphere of competence, code of conduct and standards of practice at all times. This includes assisting registered practitioners in the provision of high standards of patient care and working as part of a team to provide high quality and compassionate care to individuals.

To carry out well-defined routine clinical duties by following care plans, for example, monitoring an individual’s conditions, checking on their overall progress, comfort and wellbeing and supporting individuals where necessary to eat, drink, wash, dress, go to the toilet and engage in rehabilitation.

To prepare individuals for healthcare activities carried out by other members of the healthcare team, looking after them before, during and/or after those activities in line with their care plan.

To identify and report any work related risks in accordance with Trust policy, utilising the appropriate reporting mechanisms, cooperating with those with designated responsibility in the investigation and management of identified risks.

To take all reasonable care for own health and safety and that of others, by ensuring equipment is used safely, is well maintained and in good working order, by attending mandatory and core training as identified within the organisation and by adhering to LCHS policies, procedures and guidelines, which are all available on the Trust’s website.

To participate in activities to improve the quality, productivity and effectiveness of care in response to local and national policies and initiatives.
