This is an exciting and unique opportunity to be at the forefront of the delivery of our crisis response teams. This is a great opportunity for a Advanced Clinical Practitioner (ACP) to utilise their transferable skills whilst increasing knowledge and skills in the management of higher acuity patients in the community on the virtual ward, home first and urgent community response pathways. We are seeking to appoint a 2.0 WTE Advanced Clinical Practitioner AHPs.
The team operate 7 days/week from 08:00-22:00 and the primary function is to reduce hospitalisation rates and support earlier hospital discharge for adults, including the provision of an urgent community response and the virtual ward. The team consists of advanced clinical nurse practitioners, nurses and clinical support staff as well as therapists and social support. Referrals come from: self referrals, primary care, community rehabilitation, ambulance service acute wards, and adult social care.
The Advanced Clinical Practitioner will be knowledgeable, and educated member of the team who is able to work autonomously making high level decisions to treat health and rehab needs. ACP’s within UCR are all also actively involved in service development roles and quality improvement as well as clinical work to meet the development requirements of the four pillars.
