CRHT are able to offer all the benefits of a hospital admission whilst remaining at home, including home visits from our experienced multi- disciplinary team.. We are committed to a focused recovery model. This post involves 7 day week 365 days per year working on a rota basis.Main duties of the jobYou will be an excellent communicator, demonstrating empathy and facilitating an inclusive and collaborative environment and assessment to those service users accessing CRHT services with a particular focus on:Utilising your excellent clinical skills and judgement to assess individual needs and risksDemonstrate an understanding of clinical interventions relevant to the care group to include education, care plans and supportive discharge planning,Work compassionately and creatively to identify therapeutic and evidence-based outcomes and care plans for people who are experiencing a crisis in their mental healthWork closely with carers, families and partner agencies to facilitate and promote the safety and the well-being of service users and their families/carers.work collaboratively across the urgent care services being responsible for supportive discharge assessments and participating in multi- agency meetings.About usWe provide mental health and learning disability care for children, young people and adults across Sussex and for children and young people in Hampshire.Working in Sussex:Travel easily between coast and countryside, with a blend of picturesque villages and seaside townsEmbrace the city life with great access to visit Brighton and LondonWith easy access to Gatwick and Heathrow and excellent railway links across the UK there is plenty of opportunity for adventureWe'd love you to join our organisation that is rated 'good' overall and 'outstanding' for caring by the CQC.
