Assistant Practitioner, Band 4, Idss East

The Intensive Dementia Service (IDS) is a countywide crisis service designed by Somerset NHS Foundation Trust for our patients with dementia. The East team is based in Yeovil. The service has been designed to provide reactive and responsive support and treatment to patients at points of crisis, with the aim of preventing admission or readmission into our Older Persons Mental Health Ward.
In order to deliver this innovative way of working, we have exciting opportunities for an Assistant Practitioner to join the team. You will have a genuine opportunity to help with ongoing development and promotion of the service, providing you with a unique opportunity to develop your skills.
As a minimum you will have a level 5 vocational qualification such as a Foundation Degree or other Level 5 in Healthcare or be currently working towards this qualification. (If you are working towards this qualification you would be employed as a Band 3 pending successful completion of the course). Experience of working in a community setting is desirable but not essential.
Assistant Practitioners will develop additional skills and knowledge based on the work of the Intensive Dementia Support Service to support the Registered Professionals within the team including initial assessment, risk assessment and management, formulation of behavioural support plans, and to demonstrate the ability to react to the changing needs of our patients.
Assistant Practitioners work at a level above that of healthcare support workers and have a more in-depth understanding of factors that influence health and ill-health.

Assistant Practitioners work as part of the Intensive Dementia Service and have direct contact with patients, service users or clients providing high quality and compassionate care. Assistant Practitioners work at a level above that of healthcare support workers and have a more in-depth understanding about factors that influence health and ill-health.
Assistant Practitioners will develop additional skills and knowledge based on the work of the Intensive Dementia Service to support the Registered Professionals within the team to provide assessment and treatment and to prevent hospital admission for patients on the caseload.
