**Please note - this vacancy could be based in the Yeovil side of the County.**

**What's the role?**

We are looking for a dynamic and inspiring practitioner to join our Discharge to Assess service, part of the Intermediate Care service for Somerset.

You will work within patients own homes to carry out specific assessments which will include goal setting, treatment planning, assessing risk, promoting and enabling independent functioning for a delegated caseload with the support of qualified staff as required. You will be committed to a person-centered approach to your work, have an ability to prioritize and work flexibly.
