**Job summary**:
If you are a Band 6 Physiotherapist with extensive experience of working in the health care setting, this is a fantastic opportunity to develop leadership and management skills to drive excellence and promote rehabilitation within South Somerset Intermediate Care Services. An exciting and unique opportunity has arisen for a proactive, dynamic and inspiring Band 6/7 Physiotherapist to join the Reablement Team covering the South Somerset Intermediate Care Beds, including Cookson's Court, Sherborne House and La Fontana.

This will be a 12-month secondment to cover maternity leave and hours will be split between the Band 7 team lead role and Band 6 physiotherapy role, with the rare opportunity to develop your skills alongside an established part time Band 7 Physiotherapist. The secondment would aim to start November 2024.

Working in partnership with an established part time Band 7 Team Lead, you will be responsible for providing clinical leadership and operational management for the South Somerset Bedded reablement Team. You will provide high quality patient-centered care and specialist rehabilitation to adults referred to the Intermediate Care Bedded Pathway Service and take a lead role in the development of the service in collaboration with other Band 7 Team Leads and the Intermediate Care senior management team.

**Main duties, tasks & skills required**:
To support with the leadership and management of the operational services of the South Somerset Intermediate Care beds.

Responsible for the delivery and maintenance of high quality evidence based care, focused on meeting the needs and choices of patients, carers and families.

To be an autonomous practitioner responsible for the assessment and treatment of patients with highly complex health and social care needs.

Provide support to Intermediate Care Bedded Flow Pathway Manager, to accept delegated responsibility for specific tasks.

The post holder will be required to work their hours over 7 days, including out of hours, weekends and bank holidays, to meet the needs of the service.

Must be a car driver with a valid driving licence or have access to transport with appropriate business insurance in order to travel throughout the Trust, to meet the needs of the service.

**Qualifications**:
**Essential**:
Degree/Diploma in Physiotherapy

Registered with Health Professions Council (HCPC)

Minimal 1 years experience as Band 6

Evidence of CPD maintained in a portfolio including attendance at recent postgraduate courses

**Desirable**:
Relevant post graduate Training Course

Leadership/management qualification

Membership of the Chartered Society of Physiotherapy

Membership of relevant CSP special interest group

**Experience**:

Proven post graduate experience with extensive clinical experience relevant to the post

Knowledge of clinical Governance

Experience of a wide range of clinical rehabilitation areas of practice to include elderly patients with complex multi-pathology, neurological, orthopaedics etc

Experience of working as part of a multidisciplinary team

Experience in a Community setting

Experience of lone working

Experience of service development and review

**Desirable**:
Experience of leading a team, eg managing AL/sickness

Working knowledge of people management responsibility and policies

Understanding the basic principles of budget management
