Rehabilitation Assistants working within the Community Intermediate Care team have a rewarding and varied job role. They visit patients in their own homes and play a fundamental part in improving the lives of people when they are at their most vulnerable.
We are looking for enthusiastic applicants with experience of working in a health and social care background and multi-disciplinary working. The post holder will provide assessment and rehabilitation to patients in their own homes and ensure patients are referred on for longer term needs.

**As a rehabilitation assistant you will**:
- Support the team to avoid patients being admitted to hospital by providing short term rehabilitation and support at home
- Provide rehabilitation for patients who have become deconditioned being in hospital to ensure they are discharged from hospital in a timely manner
- Assess and provide suitable equipment to enable people to live independently
- Be accountable for providing quality care to our patients
- Utilise technology to enable you to update records as you go
Patients treated by this team are often complex and require a multidisciplinary response to treat and rehabilitate them in their own environment. With a fantastic ethos our dedicated team has a passion for quality care.
Must have skills
You will need to adapt to different scenarios in the community and can work with a range of health professionals and informal carers. You will need to be able to always drive and have access to a car for work purposes to enable you to fulfil your community role.
You will be part of a dedicated team who have a passion for quality care and should be able to work flexibly with mínimal supervision.
