We are a patient centred charity that delivers palliative and end of life care to thousands of people each year with advanced life-limiting illnesses who live in Northwest Surrey. The Hospice also provides support to their carers and families in patient homes, in care or nursing homes, or through its In-Patient Unit at the Hospice, as well as through the Bradbury Wellbeing Centre. The role of the Band 4 therapy assistant is an essential component in supporting and complementing the Hospice therapy team ensuring that the highest standards of care are implemented and that services meet statutory regulations and national standards. Job Summary To work as part of the WSBH therapy team to support the Physiotherapists and Occupational Therapists in their duties, and contribute to the holistic care of patients under the care of Woking and Sam Beare Hospices To work flexibly and collaboratively with all disciplines as part of a multi-disciplinary team (MDT) to promote a high standard of therapeutic and rehabilitative care for patients under the care of Woking and Sam Beare Hospice.

This will involve work across community, inpatient and Wellbeing services. To be responsible for a delegated caseload of patients who have had an initial assessment by a qualified therapist, and to provide follow-up visits, equipment provision and advice to patients and their families / carers as appropriate To undertake skilled and effective communication with patients and their relatives as well as the wider multi-disciplinary team To complete onward referrals as required and communicate well with external and internal agencies to ensure patient-centred care at all times
