A Community Mental Health Nurse will act as an effective member of the integrated Community Mental Health Rehabilitation & Enablement Service (CMHRES), with responsibility for a defined caseload of service users who have complex mental health and rehabilitation needs. The main aim of the Team is to focus on rehabilitation, engagement, relapse prevention and enablement to optimise the service users ability to live independentlyand enjoy a good quality of life.

Rehabilitation, relapse prevention, education and advice on the warning signs of relapse is an integral function of the post. As a specialist service liaison with other agencies and workers in the statutory, voluntary, welfare and housing sectors is essential in order to promote the most efficient use of services for service users through coordinated case work advice and consultancy.

To work effectively within the Multi-Professional team to deliver high quality care. Act as Care Programme Approach (CPA) care co-ordinator for an average of 15 service users. The rational for limiting the number of service users to each worker is to facilitate the intensity of support and input each client requires and utilise a Team approach in terms of a sharedcaseload. Maintaining appropriate, accurate written and computerised records is essential to ensure the safety of the service users and staff.
