Job Introduction
**Are you looking for an opportunity to be part of a transformational community team?**

We have an exciting opportunity for a **Therapy Assistant Practitioner **to join our supportive multi-disciplinary Intermediate Care Team across BaNES.

The service operates **core working hours of 08:00 to 16:30 Monday to Friday**. At this time, the team does not provide a service on weekends and Bank Holidays however this may change in the future dependent on demand. **Flexible working patterns** will be considered on a case-by-case basis ensuring the needs of service can be safely met.

**Your base will be within Charlton House in Keynsham**. Colleagues may also work from any HCRG Care Group locality base. Visits will be carried out across the whole BaNES geographical area.

The Intermediate Care Team as part of Reablement, is a service which supports and works with people staying in the Discharge 2 Assess (D2A) beds. The aim of the service is to **maximise independence through the provision of rehabilitation and support**.

Main Responsibilities
The post involves working with people with a **range of physical and psychological conditions** and diverse needs within the context of a community-based multidisciplinary team.

The post holder will be expected to:

- Support clinicians with their assessment and intervention plans
- Hold and manage their own allocated patients and diary
- Work regularly without direct, or under mínimal supervision
- Provide rehabilitation and support as identified. This may involve delivering equipment items, promoting independence in daily living tasks, including personal care, washing and dressing, meal preparation, general mobility and transfers.
- Encourage rehabilitation approaches to restore independence
- To carry out basic nursing care tasks as necessary, including continence management and the prompting of medication
- To feedback progress of service users, liaising with colleagues and other agencies and professionals in order to provide a co-ordinated service. This might include care home staff, GP’s, district nurses, social workers and family and carers.

You will have experience of working in a care setting e.g. care home, hospital, community care or rehabilitation alongside knowledge regarding rehabilitation and the importance of maintaining independence.

Due to the community nature of this post, you will require a Full UK Driver’s License and have access to a car / vehicle.

You will also need to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i.e. Outlook, Word, Excel.
