**JOB PURPOSE**:
Our specialist Springboard provision is based in two offsite centres, with one team in Letchworth and another in Watford. The Springboard centre and transition coordinator role would be based in Letchworth.

Springboard is a full-time personalised and therapeutic learning programme for young people aged 16-24 who require additional support before progressing to a mainstream course or employment. Many of our students have a diagnosis or traits of high functioning Autistic Spectrum Condition with associated emotional, behavioural and mental health needs. The discreet S.E.N.D. provision at NHC, including Springboard, was assessed as Outstanding by Ofsted in 2017.

**We are looking for a highly organised, innovative, team player who is motivated and committed to the success of students with SEND.**

To maximise referrals and assess suitability of the provision for students with complex needs, to ensure outstanding achievement on programme and a successful transition onto Further Education or employment.

**PERSON SPECIFICATION**
- At least 2 years’ experience of working with and assessing young people with High Functioning Autism and/or Social, Emotional and Mental Health difficulties, ideally in an educational setting
- Outstanding written and verbal communication skills with the ability to build and maintain relationships at all levels
- Excellent organisation skills and the ability to manage a complex workload effectively
- Knowledge of SEND reforms, High Needs Funding and the impact of Education, Health & Care Plans
- Minimum Level 4 qualification or minimum 2 years’ experience in a related field
- Level 2 qualification in maths and English or equivalent
- This role is based in Letchworth but on occasions may involve working across group sites in North Herts.

**ROLES AND RESPONSIBILITIES**
- Work collaboratively with schools, parents/carers and other involved professionals to support young people with High Functioning Autism and associated Social, Emotional and Mental Health difficulties with their transition to and from Springboard
- Undertake detailed assessment of students in order to determine suitability for Springboard and maintain accurate records of referrals storing background reports centrally and confidentially in accordance with GDPR
- Recruit to agreed target numbers, with a view to growing the provision, to meet need
- Complete documentation detailing individual programmes and additional High Needs Funding required to meet needs, submitting to Local Authority within deadline
- Coordinate the preparation of Education Health & Care Plan (EHCP) documentation and lead annual review meetings
- Liaise with Springboard team to ensure the individual needs of students are met, including safeguarding requirements, through sharing of information e.g. detailed profiles
- Coordinate individual student programmes and monitor effectiveness in close liaison with Springboard Manager, Teaching & Learning delivery team, therapists, external agencies and parents
- Plan and support effective transition from Springboard to future placement by ensuring detailed information concerning support needs is shared
- Key work a cohort of students to review progress towards programme targets and EHCP outcomes
- Provide occasional in class support for individuals, to ensure a smooth transition and high levels of engagement

**Job Types**: Part-time, Permanent
Part-time hours: 29.6 per week

**Salary**: Up to £25,600.00 per year
