Position: Support worker
Responsible to: Case Manager, Registered Manager
Renumeration: £11.00ph

**Purpose**

The purpose of the Support Worker role is to assist our service users to remain as independent as possible in their own homes and to achieve their goals, which are personal to them.

Our Support Workers deliver care in the home and other venues and work to the bespoke support plans which have been fully designed with our service users.

Duties and tasks will include the provision of personal care, administration of medication, support with fluids and nutrition, domestic tasks, enabling and support to integrate in and contribute to the society in which they live.

**Principal Responsibilities**

1. To deliver care to service users which is of a high quality and is in line with the values and promises of Sphere Specialist Healthcare.

2. To encourage service users to remain as independent as possible.
3. To be aware of the personal needs and preferences of your service users.
4. To communicate appropriately and sensitively with our service users using a variety of methods depending upon service users’ needs and preferences.
5. To work in a way which promotes dignity, respect and privacy of service users and their families and friends.
6. To recognise that service users requiring care and support will impact upon those around them, being sensitive to this ensuring the service user’s family and friends are supported in the process, communicated with sensitively and treat with dignity and respect.

7. To liaise with the Multi-Disciplinary Team involved in the care of the service user, ensuring all involved in the care are aware of all relevant information to their role. Drawing upon the knowledge, skills, and expert advice from health professionals in their respective field.

8. Deliver care and support in a manner which is sensitive to the service user’s culture, disability, race, age, sexuality, marital or civil partnership status, gender status and identity or religion/belief.

9. To support service users to remain a part of the community within which they live by supporting them to integrate into and contribute to the community.

10. To assist service users with mobility problems and other physical disabilities and help in use and care of aids and personal equipment.

11. To engage in the promotion of mental and physical wellbeing of service users through talking to them, taking them out and sharing with them in activities, which have been agreed as part of the ongoing care planning process.

12. To support service users to work towards goals which have been agreed as part of the care planning process and feedback when something is going well or not so well. Working with the team when something is not going so well to adapt practice to give the service user the best chance of meeting their goals.

13. To read and write reports and take part in staff and service users’ meetings and in training activities as directed.

14. To always comply with the organisation’s policies and procedures, which includes the service users care plan and medication needs.

15. To report to the registered manager any significant changes in the health or circumstances of a service user.

16. All staff are required to attend mandatory training where appropriate; this includes induction and National Vocational Qualifications or their equivalent.

17. To enable and assist service users to maintain on a day-to-day basis as much autonomy and choice as possible. Supporting service users to gain knowledge when possible, to enable them to participate more and more in the decision making process and have control over their life choices.

18. To support and assist service users in identifying risks and helping them to navigate mitigating those risks to ensure they are safe and happy. This may involve supporting and managing positive risk taking in a manner which ensures all planning and measures are in place to allow this to occur safely.

19. Identify and support appropriately service users with capacity and those who lack capacity.
20. Record appropriately all care given which relates to service users who lack capacity.
21. Involve and consult with service users, their families, representatives or advocates to ensure that the service user’s views are expressed and acted upon.
22. Update and continually improve practice by a mutually agreed method which may include attendance at staff meetings, learning support briefings and one-to-one supervision.
23. Take responsibility in conjunction with line manager for identifying further learning.
24. Be aware of requirements regarding codes of practice and relevant codes of conduct where appropriate issued by professional bodies.
25. Understand the requirement of the New Fundamental Standard Regulations 9 - 20, in relation to the Care Quality Commission Inspect regime and its importance to your workplace practice.
