**Gateway Support and Care Ltd is a supported accommodation service offering housing with care and support for individuals with complex needs including mental health, learning difficulties, substance, and alcohol misuse, who may be homeless, rough sleeping living in hostels, at risk of eviction or in hospital settings.**
**This is a challenging but rewarding role where no two days are the same. You may be faced with challenging situations, unusual, anti-social and offending behaviours.**
**We are seeking a compassionate and dedicated Support Worker to join our domiciliary care team in Wales. In this role, you will provide high-quality, person-centred care to individuals with a variety of needs, including mental health conditions, learning difficulties, and other complex support requirements. Your work will be focused on enhancing the well-being and quality of life for our service users, assisting them with daily activities and promoting their independence in their own homes.**
**Duties and Responsibilities**:
**Client Care & Support**
- Provide assistance with daily living activities, including meal preparation and medication management.
- Offer emotional and practical support to individuals experiencing mental health challenges, substance misuse, or learning difficulties.
- Support individuals in maintaining or developing independence, and help them engage in social, educational, or recreational activities.
- Assist in managing day-to-day life, including budgeting, shopping, and accessing services, to help individuals build life skills and gain confidence.
- Monitor and document service users' well-being, behaviour, and progress, and promptly report any concerns to the senior support worker or care manager.
**Person-Centred Care & Support Plans**
- Work with service users to create and implement personalized care plans tailored to their unique needs and preferences.
- Review care plans regularly, ensuring they are updated to reflect changes in individuals' circumstances or needs.
- Encourage individuals to participate in goal setting, empowering them to make decisions about their own care and support.
**Health & Safety Compliance**
- Ensure that the service provided adheres to health and safety guidelines, safeguarding policies, and other relevant regulations.
- Assist individuals with medication as per their care plans, ensuring correct dosage and proper documentation.
- Monitor and report any health changes or concerns and assist individuals in accessing healthcare services if needed.
**Specialised Support for Complex Needs**
- Provide specialized support for individuals dealing with substance misuse, mental health challenges, or homelessness, helping them engage with relevant support services.
- Assist with accessing accommodation or housing services for individuals at risk of homelessness or rough sleeping.
- Offer support to individuals transitioning from hospital settings, ensuring continuity of care and helping with reintegration into the community.
- Work alongside other professionals, such as social workers, mental health teams, and substance misuse services, to ensure coordinated care and support.
**Key Skills & Competencies**
- Compassionate, empathetic, and non-judgmental approach to working with individuals who have complex and challenging needs.
- Strong communication skills, both written and verbal, for interacting with service users, families, colleagues, and external agencies.
- Ability to remain calm and supportive in challenging situations and offer practical solutions to problems.
- Knowledge of safeguarding procedures, health and safety regulations, and care standards.
- Flexibility and adaptability to work with individuals in a variety of living situations and circumstances.
**Qualifications & Experience**
- Level 2 or Level 3 Diploma in Health and Social Care (or equivalent qualification) is preferred.
- Willingness to register with Social Care Wales.
- Experience in working with individuals with complex needs, including mental health conditions, learning disabilities, alcohol and substance misuse, and those experiencing homelessness.
- Understanding of Welsh social care regulations, including the Regulation and Inspection of Social Care (Wales) Act 2016 and Care Inspector of Wales (CIW) standards.
