This role is an exciting opportunity to influence clinical teams to develop a truly patient and carer focused, partnership approach to delivering care.
A carer is someone who provides unpaid support to family or friends who could not manage without this help. This could be caring for a relative, partner or friend who is ill, frail, disabled or has mental health problems.
This role will develop and deliver the support offered to unpaid carers when the person they care for is admitted to our acute and community hospitals.
We are looking for a confident, creative, enthusiastic, self motivated and compassionate individual who has experience of working in partnership with people.
Ideally you will have experience in healthcare, working with unpaid carers or have personal experience of being an unpaid carer.
The role will be part of the Patient Experience team and you will work with colleagues to ensure that the voice of patients, services users, carers, families and the public has an influence on service development and improvement. Working hours: Monday - Friday 8:30 - 4:30 Occasional evenings and weekend to support patients experience activities.

This role is essential to supporting and involving carers across Somerset NHS Foundation Trust.
**The purpose of the role will be**:
To be the first point of contact for the day to day operational work of the involvement team with the specific focus on the involvement, support and guidance for the carers of patients who are receiving care in acute and community settings. Liaising with colleagues across the organisation, carers, patients, families, external organisations and the public to ensure that the carer voice is heard across the trust.
To participate in the coordination of a range of projects regarding unpaid carers, volunteer, patient and public involvement and feedback.
To be responsible for maintaining various databases with up to date information and participate in essential administrative tasks to support the patient, carer and public involvement agenda.
To provide support and advice to unpaid carers and signpost them to appropriate services.

- To support patient, service user, carer and public involvement across the organisation.
- Support clinical staff to identify unpaid carers and offer support, advice and guidance to carers and clinical staff.
- To be the point of contact for carers for support whilst the person they care for is in hospital and provide signposting to relevant services.
- To provide administrative duties relating to carer and service user involvement, Volunteers, patient and family feedback activities
- Facilitate and develop the supporting and involving carers’ project.
- Generate, organise and maintain an accurate record of carers, patient partner/ service user and Volunteer involvement.
- Support with the patient and carer feedback agenda and provide administrative support with the development of different approaches to gain feedback from patients.
- Support the Involvement lead with the management and coordination of patient experience projects.
- Outreach to wards and teams across the organisation to support carers and to facilitate the gathering of and learning from patient and carer feedback.
