Adult Speech and Language Therapy

1.0 wte Band 5 SLT post

We are looking for proactive, flexible and enthusiastic SLT who demonstrates skill, knowledge and evidence-based management of adults with communication and swallowing disorders. We can offer robust support and supervision to newly qualified SLTs and in house dysphagia training.

Our post provides an exciting new opportunity to develop clinical and caseload management skills on the in-patient medical and stroke wards. The post will be based at Chesterfield Royal Hospital.

Adult Speech & Language Therapy is located within Therapy Services and is well supported by a dedicated administration team. SystmOne is used across Therapy services. We have close working relationships across multi-disciplinary teams e.g. Critical Care, Stroke Unit, ENT and Head and Neck cancer.

Chesterfield Royal Hospital NHSFT is a busy district general hospital, just off junction 29 of the M1, within easy reach of Sheffield, Nottingham and Derby. Staff wellbeing is promoted and supported within the Trust.

To work as part of the Speech and Language therapy team for Adults providing services to the Trust.

The post holder will provide a service to patients referred to the department with communication and swallowing disorders, this will include inpatients and outpatients.

The Adult Speech & Language Therapy Teamcontributes to the delivery of outstanding patient care by:

- Providing timely patient centred interventions for communication and swallowing difficulties
- Supporting functional recovery and /or quality of life for patients and their carers.
- Working closely with other members of the healthcare team within and external to the Trust to support care delivery
- Advocate for these patients

**Clinical responsibilities**:
To provide specialist assessment, differential diagnosis, management and treatment to patients with acquired communication and swallowing disorders.

To take a holistic approach in developing clear care plans based on knowledge of evidence based practice, ensuring patients, carers and other professionals are involved.

To devise and implement a range of group and individualised therapy programmes.

To work as part of the multi-disciplinary team, liaising and providing support and guidance on clinically relevant issues.

To sensitively communicate complex patient condition information to patients/carers, families and members of the MDT and other professionals.

To manage own caseload and clinical priorities, with guidance from the senior clinicians.

To contribute to the development of evidence based resource and knowledge for the department; fostering innovation and continuous service improvement.

To provide effective and appropriate support, supervision and guidance for less experienced therapists and SLT assistants.

To provide clinical training of SLT students and support staff.

To provide training to other staff, members of the public and involved professionals.

To participate in research development and clinical audit in the area of work.

To actively participate in regular supervision and demonstrate the ability to reflect on practice with peers/clinical supervisor.

To be accountable for own professional actions and recognise own professional boundaries, seeking advice as appropriate.

**General responsibilities**:
To ensure all documentation complies with professional standards and an accurate clinical record is available for each contact on the electronic patient record system

To maintain confidentiality at all times relating to the patients, staff and the Trust.

To actively participate in the Staff Appraisal process.

To ensure accurate and timely data collection according to Trust requirements.
