**Band 6 Community Mental Health Occupational Therapist**:

- **Start ASAP - Ongoing**:

- To manage a caseload of clients with Occupational Therapy needs, using evidence-based / client-centred principles to assess, plan, implement and evaluate OT interventions to mental health service users on the community mental health team caseload in communityand home settings (including contact with carers)
- To provide individualised or group interventions appropriate to the clients on the caseload which support their OT recovery goals and cover a range of functional and daily life skills needs.
- To liaise with Croydon Social services departments and other external agencies including GPs to facilitate clients access, support of needs, physical disabilities and discharge planning
- To liaise with community resources in the social care and voluntary sectors and support clients to access these resources.
- Some OT therapeutic group work/ group interventions in collaboration with other CMHT OTs may be required.
- To participate in the planning, development and evaluation of Community OT / MDT services, and being involved in defined projects and activities
- Some MDT/generic teamwork may be required (such as duty support).
- To support care co-ordinators under the CPA process when necessary (not routinely part of the role)
- To work alongside a Band 5 OT and in the team and provide support and guidance regarding managing the OT caseload in the team.
