Pay Rate: up to £35 ph
We specialize in facilitating safe transitions of care from hospital to the community, ensuring individuals long-term care needs are met while promoting independence. Our team works closely with multi-disciplinary partners in a fast-paced environment.
As a Level 1 Hospital Social Worker, you will manage a diverse caseload focused on capacity assessments, best interest decision making, and Care Act assessments. Youll be an integral part of our duty rota, delivering exceptional social care to adults and their carers while championing choice and control.
Key Responsibilities:
Conducting capacity assessments and best interest decision making.
Performing Care Act assessments and contributing to duty rota duties.
Delivering high-quality social care to adults and their carers.
Promoting choice, control, and independence in care planning.
Engaging in strengths-based, person-centred assessments.
Facilitating outcomes-focused and preventative conversations.
Supporting hospital discharges primarily from Pinderfields or Dewsbury District Hospitals.
**Requirements**:
Qualified Grade 8 Social Worker with relevant experience.
Enthusiasm for strengths-based, person-centred approaches.
Ability to thrive in a dynamic, team-oriented environment.
Strong interpersonal and communication skills.
Dedication to promoting independence and well-being.
