Autonomously act and be recognised as a clinical expert in their specialist field using developed specialist clinical knowledge ensuring patient/client centred care and family involvement, including health promotion and disease/accident prevention. Make difficult decisions and encourage critical thinking regarding both ethical and legal issues related to care delivery. To manage own case load (whilst acting as an Independent Prescriber) that will often be complex and unpredictable: Receive referrals, assess, order, interpret and act upon investigative tests, consider differential diagnosis, evaluate, treat, plan, refer and discharge patients/clients within the Trust Develop a robust framework of clinical competencies for staff working within the team and be responsible for implementing evidence-based group education models into all aspects of care delivery where appropriate including structured education programmes Maintain own clinical development by keeping abreast of new treatments and technologies and by ensuring care is evidence-based To improve the quality of the patient experience by identifying and meeting the individual clinical needs of patients Exhibit a commitment to new ways of working across Community services To provide clinical advice and support for ACPs and trainee ACPs. To develop and deliver educational training and development to the ACPs on a regular basis Ensure compliance with policies, procedures and clinical guidelines for self and others, participate in the development of same.
Management and Leadership To provide effective professional leadership of the ACPs across community services. To identify and lead changes in practice and service delivery in accordance with evidence-based standards and policy, challenging traditional boundaries and current thinking and overcoming resistance and conflict at Trust level. Maintain a high profile within the Trust and externally, providing a strong contribution to the achievement of organisational goals. Provide support and supervision for senior staff working towards a higher level of practice.
Exhibit the highest level of interpersonal and communication skills and develop effective communication strategies with other members of the multidisciplinary team Lead, facilitate, enhance and develop appropriate Advanced Practitioner skills and expertise through regular supervision in accordance with professional practice guidelines. The post holder is required to provide, analyse and receive highly complex and sensitive information. Use his/her expert knowledge and experience to provide professional advice and support to nurses, and other staff and managers across Community Services. Challenge practice standards and influence constructive and positive change.

- governance in accordance with professional codes of practice of the Nursing and Midwifery Council and Trust policies and procedures. If successful this vacancy will require an enhanced DBS check at a cost of £54.90. This cost will be deducted from your first Trust salary In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16.
