An exciting opportunity has become available to work within our growing Venture Onward Service.
We have full-time or part-time roles available for Mental Health Reablement Workers.
We provide bespoke, community-based support packages to individuals with a complex need, including a mental health diagnosis and Autism Spectrum Disorder. Our clients may have spent an extended period on hospital or other setting. We work with a person-centredapproach; empowering individuals to become as independent as possible within their own home.
Your role will be delivering quality support that enables people to achieve their outcomes by using and developing person-centred support plans. You will be able to manage risks and vulnerabilities whilst promoting social inclusion and maximising the opportunitiesfor the people you support.

**Essential Skills**

**Desirable Skills**
It would be desirable for you to have had experience of supporting a person(s) with a mental health condition and within a community-based services.
