Rate - £11- £12 PAYE
- To provide a full administration service to the CHC team. From initial referral, throughout the eligibility process and to support the requirements post panel administration
- To log referrals in order of priority according to process and create accurate patient records on the electronic system
- To input data on electronic system, to enable accuracy in reporting
- To liaise with families or representatives, ensuring they are kept fully informed and up to date throughout the CHC process
- To receive sensitive information via patient notes and reports from GPs, hospitals, care homes, Social Services, district nurses and other health professionals along with submissions from individuals and their representatives
- To request, monitor and respond to stakeholders to request for information, communicating and escalating to line managers where necessary to meet strict timeframes
- To be responsible for photocopying and checking legal documentation e.g. Power of Attorney and identification documents and returning them to sender
- Compile and maintain electronic individual files for all cases and source additional information as required to enable the CHC process to progress
- Ensure that patient confidentiality and the sharing of records is according to relevant legislation e.g. Data Protection Legislation, Information Governance and Caldecott Guidance
- Liaise with senior staff to ensure cases are allocated to professional staff to enable their completion
- To liaise with patient/representatives and stakeholders to arrange appointments and the bookings for Continuing Healthcare assessments
- To undertake general office duties including filing, processing incoming and outgoing post as well as scanning and photocopying
- Monitor progress of cases and liaise with senior staff to ensure agreed timescales are adhered to
- To prioritise panel cases for a formal decision and record information onto the database
- Responsible for planning and organising all CHC panels in advance. Coordinating panel representatives, Multi-Disciplinary Teams (MDT's) and the gathering of all relevant information within the correct timeframe
- To ensure all relevant patient information including patient case summaries is provided to NHS England for the Independent Review Panels
