**Job summary**: The post holder is a Nursing Associate registered with the Nursing and Midwifery Council. They are required to act within their professional boundaries, supporting and working alongside a multi-disciplinary team within general practice and a Primary Care Network (PCN). You will be an integral part of the nursing team working with people of all ages. You will be an experienced practitioner who is comfortable working independently and under the leadership of our Registered Nurses, supporting with all aspects of nursing care and improving on the safety and quality of patient care at every opportunity.

The post holder will work with other members of the Multidisciplinary Team (MDT) to both organise and facilitate a high professional standard of care delivery in adherence to all practice protocols/policies & guidelines, appropriate to the individual needs of the registered patient. Main duties: are underpinned by the six platforms of good practice. Being an accountable professional: provide safe and compassionate care thats based on a persons unique needs, act professionally at all times and take responsibility in making informed decisions within your scoop of clinical practice. Promoting health and preventing ill health: be aware of the public health agenda to support patients in managing their health so that they can have a good quality of life.

Contribute to the achievement of local and national quality improvement schemes. Providing and monitoring care: assess and evaluate patients physical and mental health needs, and work with them and their family or carers to support shared decision making in care delivery. Working in teams: work alongside a multidisciplinary team within the practice, the Primary Care Network (PCN), community, secondary care trusts and voluntary sector. Improving safety and quality of care: help monitor and improve the quality of care a patient receives, identify any risks to safety and take action to manage these, ensuring a patients best interests, needs and preferences are put first.

**Contributing to integrated care**: through excellent communication and organisational skills have a key role in the provision of care, alongside a range of other professionals and carers. Job responsibilities: Assistand perform routine nursing tasks related to patient care, such as routine BP checks, ECG, suture and clip removal, BMI and lifestyle advice, as directed and agreed with line manager, registered general nurses and GPs. Supportand signpost patients to improve and maintain their mental, physical, behavioural health and wellbeing by using behaviour change interventions, such as undertaking health checks (physical and mental), Undertakeadult NHS immunisation programmes and ensure vaccines/injections are administrated under patient group directives or patient specific direction. Undertake administration of prescribed medicines via oral, IM, SC routes, following competency based training and practice policy.

Provide holistic assessment and treatment of patients attending for ear irrigation following relevant training. Undertake the collection of pathological & microbiology specimens including venepuncture, swabs, urine culture in line with local guidelines Support chronic disease management including monitoring patients with stable asthma, undertaking preliminary investigations for Type 2 diabetes patients, such as foot checks. Provide general health promotion education, advice on national screening programs and guide/refer patients to appropriate services. Undertakescreening programs such as cervical screening tests.

Undertake any investigatory test, such as Ambulatory BP monitoring, automated Dopplers and Spirometry, taking basic history and referring patients to relevant clinician for interpretation/ planning of care. Provideappropriate self-care advice for patients presenting with minor illness within own level of competency and referring to GP/ANP where appropriate. Chaperone and assist patients where appropriate who are being examined by another clinician. AssistGPs with minor surgery and well patient procedures.

Maintain effective infection control measures in line with local guidelines and practice policy. To be able to recognise and manage anaphylaxis, according to the current UK Resuscitation Council guidelines. To be able to perform Cardio-pulmonary resuscitation (BLS) and the correct use of Automated Defibrillator following training, according to the current UK Resuscitation Council guidelines. Engage in the support of learners for example nursing and medical students, and newly appointed colleagues.

Ensure safe storage, rotation and disposal of vaccines and medication. Assist in the completion of monthly stock take and support the cost-effective use of disposable items. Maintain adequate stock within the treatment room and assist with the re-stocking of other clinical rooms as required.
